What should I do if there is an emergency?
If you are experiencing a potentially life-threatening emergency, call 911 immediately. If you need urgent care treatment (for conditions such as a cold or a broken bone) during business hours, call 504.363.4711. For after-hours urgent care treatment, visit your nearest hospital. (Ochsner Medical Center – West Bank is located less than a mile from Valentine Medical Center at 2500 Belle Chasse Highway.)
What are your hours?
Business hours are Monday through Friday, 7:30 am to 5:00 pm.

What do I need to bring with me?
If this is your first visit to Valentine Medical Center, you should bring any medical records you have, as well as a list of any current medications you are taking. You should always bring your insurance card and a valid picture id.
The following forms of identification are accepted:
- Driver’s license
- Government-issued identification card
- Military identification
- Passport
How do I get a copy of my medical records?
To obtain a copy of your medical records, send a written request to the following address:

Can you please define some basic healthcare and insurance terms?
Copayment – A copayment, also known as a copay, is a flat fee that an insured person is required to pay each time he or she fills a prescription, sees a doctor, or receives medical services of any kind. The amount of your copay is predetermined by your insurance company based on the insurance plan to which you belong and can vary depending on the type of service you require.
Medicare – Medicare is an insurance program funded and administered by the federal government. It covers people who are over the age of 65 or who have certain disabilities that qualify them for coverage.
Out-of-pocket – Any fee that a patient pays for with his or her own money.
Referral – Written approval, provided by your primary care physician, that allows you to obtain specialized medical care.
